You only need to supply either your first or last name on the Renew Online page.


To Apply for DGS Membership:

  • Apply on Line - Go to Online Application
  • Download Application Form (PDF) and mail

    NEW MEMBERS:

    Once your membership application has been approved and added to the membership database, you can have your user name and a temporary password emailed to you by going to Member Log In.
